Generate plausible reasons for the failure across areas that are relevant to this plan, such as: - unclear ownership; - wrong assumptions; - timing or dependencies; - user/customer adoption; - quality problems; - resources or capacity; - communication; - external changes. For each plausible failure mode provide: - what went wrong; - why this plan is exposed to it; - an early warning sign; - one prevention or mitigation action; - severity if it happened. Then identify the 3 safeguards worth adding before execution. Do not create far-fetched disaster scenarios just to fill categories. Base the pre-mortem on the plan I provide and label speculation clearly.
